$5 Coupon Terms:
Send:
1) Proof-of-purchase (tab or UPC from your Indiana Jones and the Kingdom of the Crystal Skull DVD) OR send in your dated cash register
receipt for your product purchase of Indiana Jones and the Kingdom of the Crystal Skull DVD with purchase price circled (photocopies
acceptable).
2) The original, unused $3.00 off coupon for Indiana Jones and the Kingdom of the Crystal Skull.
3) This rebate form competed with your information.
 
I'm going to try and send it in. I bought off of Amazon, but hey the worst they can do is just deny it. I don't have to cut up by insert because it says they will accept a copy of the receipt so I'm just printing an invoice from my Amazon account.

As for the coupon just call your local Papa Johns and say you didn't get one with your last order (they told me to come pick one up). I actually didn't get one.
