I've never understood the fuss about In-N-Out. I've tried it several times when out West for business etc., and it just seems like pretty standard fast food burger to me.
Five Guys still gets my vote for best fast food burger, decent quality beef for fast food and all the topping options make a huge difference. By I seldom get fast food. If I'm going to eat unhealthy and have a burger, I'd rather just go all out and get a high quality burger at a sit down joint--preferably a local place, but in a pinch a chain like Red Robin or Cheeburger, Cheeburger.
I'm the reverse. Love In-N-Out but can't believe 5 guys is mentioned in the same breath. By far, the greasiest, sloppiest hamburger I've ever had. Insta- stomach ache. It is like a meatball on a wet bun.
There is a joint that started here (they are all over now) that I really like - Smashburger that is awesome
edit: looks like they have a few NYC locations
